How to Become a Member

Enrollment in ICS is voluntary. The first step is to call ICS at 212-584-2500 and ask for a member of our Intake Staff.

The enrollment process takes place over one or two interviews to make sure that the person interested is fully informed of ICS’ benefits and for a complete assessment of his or her needs, goals and preferences.

Here’s how it works.

Meeting You. Meeting Us.
When you call us, we ask a few questions to get information about your disability and give preliminary information about ICS and our eligibility requirements.

We arrange a convenient time to meet with you either in your home, hospital or nursing home, wherever you prefer. You will have the opportunity to ask any questions you may have, and our enrollment team will be able to fully explain the ICS plan of services.

Enrollment staff will ask about any preferences regarding your current service network and check your financial eligibility for Medicaid. If you need help to apply for Medicaid or recertify eligibility, the ICS Medicaid Eligibility Specialist is available to assist you.

An ICS intake nurse will meet with you to assess your capabilities and discuss your goals and preferences for care. Together, you will develop an initial care plan.

Paperwork
Once you have all the information you need and have decided to enroll, the enrollment package described above is completed.

You will be asked to sign an enrollment agreement and an initial care plan.

If additional information is needed from your medical or mental health provider, the enrollment staff will ask you to arrange for your medical records to be sent to ICS.

We will contact you for an initial orientation after becoming a member of ICS.

We will help you understand how to use the benefits provided by membership in ICS and help you learn the best way to communicate your questions, concerns, suggestions, and recommendations to ICS.

ICS staff will always listen to your concerns and complaints and be responsive to your suggestions on how we can better work with you.
