That’s what happens after you contact ICS to begin your enrollment process.
When we talk, we ask a few questions to get information about your health care needs, chronic conditions, and disabilities and to give you preliminary information about ICS and our eligibility requirements.
You will have the opportunity to ask any questions you have, and our enrollment team will fully explain the ICS plan and our services. Enrollment staff will ask about any preferences you have regarding your current service network and check your financial eligibility for Medicaid. If you need help to apply for Medicaid or recertify eligibility, the ICS Medicaid Eligibility Specialist is available to assist you.
Once you decide that ICS is the right plan for you, we arrange a convenient time for an intake nurse to come and meet with you wherever you are—at home or in a hospital or nursing home. The nurse will assess your capabilities and discuss your goals and preferences for care. Together you will develop an initial Care Plan.
When you have all the information you need, the nursing assessment has shown that you meet all the eligibility requirements, and you have decided to enroll, you will be asked to sign an enrollment agreement and the initial Care Plan.
Getting Started as a Member
To help you get started as an ICS member, we contact you for a telephone orientation. We help you to understand our Care System, how to use the benefits provided by membership in ICS, and the best ways to communicate your questions, concerns, suggestions, and recommendations to us.
Now that you are a member of ICS, you have an interdisciplinary Care Management Team responsible for working with you and any family involved in your care to identify and ensure that you receive the full range of home care, health care and social services you need to live as independently as possible and be as active as you want to be in community life.
Remember: our staff will always listen to your concerns and work with you to solve any issues that arise. You’re a member now; you don’t have to go it alone.
