Eligibility
You can become an ICS member if you:
- Live in the Bronx, Brooklyn or Manhattan.
- Have a physical disability or chronic illness that will last longer than 120 days and that requires a nursing home level of care and ICS’s services. (ICS is able to determine this; an ICS nurse will come to your home and conduct a medical assessment, using NYS Health Department guidelines.)
- Are at least 18 years old.
- Are eligible for Medicaid. (If you’re not on Medicaid, ICS will help you apply.)
- Can return to—or remain in—your community, without risk to health or safety.
Medicaid and Your Resources
You may worry about how your finances will affect your eligibility for Medicaid. This can be especially so if you work, have a home, or have other assets. We want you to know that you have new choices. The New York State government allows some people with disabilities who have assets and/or income above the usual Medicaid limit to be eligible for Medicaid, without spending down assets or income. An ICS Medicaid Eligibility Specialist will help you to understand the regulations and get you the help you need.
