Here at ICS we work very hard to make the process of joining our Medicaid managed long-term care plan for adults with physical disabilities or chronic illnesses as easy as possible.

There are certain eligibility requirements, and you can get more information or begin the application process in any of three easy ways. The steps in enrollment involve our meeting you and your meeting us and, if you think ICS meets your needs, having a medical assessment to insure your eligibility. We want to be sure that you understand the benefits and requirements of ICS membership and that we understand what services you need to live a fulfilling, independent life. Together, we create your plan of care.
