Nurse - Care Manager - RN
We currently seek a Care Manager (RN), to supervise and collaborate within a team of home care delivery professionals.
Interdisciplinary team responsibilities include needs assessment, care planning, medication management, DME use and safety and ensuring that all required documentation and filings are in compliance.
Success in this position requires strong commitment to excellent customer service, as well as the ability to manage a staff of professionals.
Requirements for this position include:
- RN with current license;
- Experience and skill in assessment and care planning;
- Excellent communication and collaboration skills.
Preferred abilities include:
- Public health, community-based or home care experience;
- Experience with rehabilitation programs;
- Ability to communicate in Spanish (English required).
