Rehabilitation Services Coordinator, Team Model
The Rehabilitation Services Coordinator is part of the interdisciplinary care management team, he/she performs activities to support the rehabilitation equipment processes related to the provision of both new rehab equipment and requests for wheelchair repair. This includes communication and coordination with members, interdisciplinary team (IDT) members, rehab department and other ICS staff members. This work is carried out in support of the mission and goals of Independence Care System. This is a full-time, permanent position.
Key Responsibilities:
1. Equipment Request through Care Compass.
Requests for New Equipment:
- Review, revise and submit rehabilitation devices evaluations request to Manager of Rehab New Rehabilitation Equipment Services for assignment/approval.
- Assist IDT team members on issues related but not limited to new rehabilitation device evaluation.
- Coordinate, maintain and distribute weekly Wheelchair Clinic Calendars to Rehab staff and suppliers.
Request for Equipment Repair:
- Respond to request from Equipment Repair indicating Wheelchair (WC) Repair needs.
- Contacting members for information, clarification, and to update on repair status; including availability of a loaner chair if applicable.
- Work with Manager of Repair Services on triaging / handling of wheelchair repair issues.
- Coordinates equipment repair with suppliers.
2. External equipment & repair evaluations
- Coordinate and document new equipment evaluation with external facilities (Mt. Sinai, ICD, Rusk).
- Generate authorizations for external wheelchair clinic visits.
3. Weekly Wheelchair Repair Workshop
- Attend weekly workshop prepared with appropriate resources and information.
- Manage the workflow between the technicians and completing documentation in Care Compass.
- Follow each workshop & process the approved orders for new parts generated through workshop appointments and track in Care Compass.
4. Track Orders
- Monitor parts & new equipment orders utilizing the established time frame expectations, and document tracking in Care Compass.
- Track notification from supplier and coordinate with Wheelchair Technician regarding pending supplies.
- Inform member of order status and/or schedule member to return to clinic/ workshop.
- Coordinate with supplier to ensure member’s parts are installed.
5. Authorizations
- Generate authorizations for review from approved supplier for new equipment and/or parts
- Attached approved authorizations along with receipt and upload them on Care Compass
- Review charges vs. payment from suppliers, utilizing appropriate resources and furnishing Finance with information/documentation for claims adjudication.
6. Deliveries
- Acts as ICS point person for deliveries from outside suppliers.
- Receive the delivery of new equipment for members prior to delivery to member at wheelchair clinic.
- Stores equipment in a secure location to be available to each clinician for check-out prior to scheduling member for delivery clinic.
- Communicates via email drop off and pick-ups to all Rehab staff on a timely basis in accordance with the delivery dates.
7. Motorized Chairs
- Monitor all motorized chairs for charging level.
- Ensure chairs are powered on a bi-weekly basis and as needed.
- Log charging history & record and tag on chair.
8. Point person to the IDT teams in the borough regarding issues related to, but not limited to, member needs for new equipment, repairs for existing equipment, order status of interventions in progress and any other rehab related needs.
9. Comply with department policy regarding the handling and storage of new and used mobility devices in or adjacent to clinical areas.
10. Other duties as assigned.
Minimum Qualifications:
- High School Diploma required or Equivalent.
- Minimum 2 years of customer service experience in a small to medium sized organization.
- Demonstrated written and oral communication skills.
- Proficient in MS Word, Excel, PowerPoint and Outlook.
Additional Qualifications:
- Associate’s Degree or higher preferred.
- Experience working with persons with physical disabilities preferred.
- Experience working in an environment with wheelchairs and/or other durable medical equipment preferred.

Senior Registered Nurse
The senior register nurse functions as a collaborative partner with the Team Leader in the Interdisciplinary Team (IDT). The senior registered nurse supports the day to day caseloads of the nurses and collaborates with other team members in addition the senior registered nurse carries an active caseload. This work is carried out in support of the mission and goals of Independence Care System. This is a full-time permanent position.
Key Responsibilities:
1. Collaboration
- Establish a collaborative working relationship with the Team Leader, the Senior Nurse and the members of the IDT
- Facilitate IDT meetings as assigned by the Team Leader
- Function as a resource for the members of the team
- Provide supervision to the Social Workers in the IDT
- Identify areas for increasing the skill level of the social workers within the IDT
- Orient and train staff as needed
- Ensure compliance with care guidelines and protocols
2. Conducts visits and assess member’s status and needs
- Assist with scheduling RN visits and assign SCICs.
- Approve increase PCA hours.
- Carry an active member caseload.
- Assess member/caregiver knowledge of necessary care needs such as medication management, DME use and safety measures.
- Collaborate with other team members and the member to establish a comprehensive care plan that identifies all needs.
- Facilitate discharge planning when necessary.
3. Documentation, Reporting and Compliance – clear and complete documentation, and reports that adhere to timeframes and ICS guidelines
- Document care plan in member’s record, member contacts, and revise written care plan as needed.
- All member-related interactions and information are thoroughly documented and reflects an understanding of member issues and needs, and appropriate interventions.
- Meets agency productivity expectations regarding all matters related to care planning and in accordance with state regulatory standards (monthly member contacts, care plans, advance directives, etc.)
- Participates in other activities related to regulatory compliance, (i.e. state audits, IPRO projects).
4. Other duties as necessary.
Minimum Qualifications:
- Registered Nurse, currently licensed to practice in New York State.
- Bachelor’s Degree in Nursing.
- Minimum six (6) months relevant work experience at ICS.
- Two (2) years of work experience in a health care setting, preferably community health
- Comfortable with extensive travel in Brooklyn, Manhattan, the Bronx, and/or Queens.
- Bilingual – (English/Spanish/Russian/Chinese or other language) a plus.
Additional Qualifications:
- Experience in program development and program management.

Senior Social Worker
The senior social worker functions as a collaborative partner with the Team Leader in the Interdisciplinary Team (IDT). Medical Coder
The Medical Coder will be responsible for ensuring accurate diagnostic and procedural coding of member medical records, claims, and Medical Encounter Data (MEDS). This work is carried out in support of the mission and goals of Independence Care System.
Key Responsibilities:
- Reviews and codes diagnoses and other procedures in accordance with established coding and abstracting guidelines (CPT, HCPCS and ICD-9 resources) as a part of medical records review.
- Reconciles medical record documentation, coding, claims and reimbursement data.
- Performs medical record reviews and assigns appropriate ICD-9-CM diagnosis codes for new and existing members based on available documentation.
- Identifies trends and ongoing problems related to medical documentation and recommends possible solutions.
- Analyzes and resolves Medicaid Encounter Data rejections related to procedure and diagnosis codes.
- Assists in the development of provider billing manuals, specifically around the procedure and diagnosis coding of claims.
- Collaborates with the business reporting manager to ensure correlation of information for rendered services.
- Makes recommendations for changes and improvements.
- Utilizes data and related modules to obtain, analyze and interpret coding, denial and other reimbursement data to support compliance and plan management activities.

Minimum Qualifications:
- High school diploma or equivalent.
- Minimum five (5) years of relevant experience.
- Knowledge of ICD-9, CPT, HCPCS and Medical Terminology
- Experience with Medicare/Medicaid/Third Part payers is required.
- Certified Medical Coder (CPC, CCS, or comparable) with high degree of competence.
- Proficient computer skills – MS Word, Excel, and Outlook.

Provider Relations Associate
The Provider Relations Associate assists in the recruitment, retention, and orientation of Network Providers by establishing and maintaining ongoing relationships with providers. This work is carried out in support of the mission and goals of Independence Care System. This is a full–time, permanent position.
Key Responsibilities:
1. Provider Network
- Establish and maintain positive and professional relationships with prospective and participating providers and outreaches according to Plan initiatives.
- Acts as liaison between provider and internal Plan departments such as Claims, Marketing, Member and Provider Enrollment and Care Management.
- Manages flow of information to and from provider offices. Monitors and communicates market trends and issues.
- Respond to provider problems and concerns; troubleshoot provider issues with Care Management, Finance and contracted providers.
- Facilitate in auditing protocol, termination, updates and addressing grievances.
- Perform site visits to ensure that facilities are ADA compliant.
- Support all credentialing and re-credentialing activities.
- Ensure coding for roper data retrieval, analysis and claims.
2. Training & Orientation
- Schedules and provides training for all participating providers as well as other organizations involved with the Independence Care system.
- Facilitate educational programs, to orient providers to ICS such as Quality Assurance protocol, Office Support Education program, etc.
- Assist in orientation and developing necessary training materials for providers of ICS’ services, including electronic presentations, provider manuals, provider directories, etc.
3. Other duties as necessary.
Minimum Qualifications:
- BA/BS Degree required in business or health related discipline such as Health Care Administration or Health Care Management.
- Minimum two years’ experience in provider relations role.
- Minimum one year experience working in a managed care or healthcare organization.
- Excellent written and oral communication skills.
- Knowledge of medical Healthcare Common Procedure Coding Systems such as, HCP, CHP, FHP, ICD-9-CM diagnosis Codes, CPT or HCPCS, unlisted, and place of service codes.
- Knowledge of Medicaid and Medicare regulations and coding.
- Comfortable with extensive travel in boroughs that ICS services.
- Knowledgeable in health care products.

Member Services Coordinator (Temporary)
As part of the care management team the Member Services Coordinator position provides timely and accurate customer service by performing activities to support the care management process and other team members, which includes assisting and educating members. This work is carried out in support of the mission and goals of Independence Care System. This is a full-time, temporary position.
Key Responsibilities:
- Responsible for answering and triaging all inbound telephone calls.
- Research and problem solve issues/inquiries. Follow up with members with the status of these issues/inquires.
- Contact Care Managers and other units as needed in order to provide members with requested information or to resolve problem.
- Monitor all open items, escalating to supervisors as necessary.
- Refer potential member leads to the Intake and Enrollment department as appointed.
- Assist members in arranging transportation or transfer to transportation department as appointed.
- Document member contacts.
- Other duties as necessary.
Minimum Qualifications:
- High School Diploma required; Advanced degree preferred.

Paraprofessional Coordinator
The Paraprofessional Service Coordinator initiates home care services for all members, and coordinates/monitor continuous home care services. S/he maintains an efficient process to ensure the delivery of high quality continuity of care for members. The Coordinator collaborates with and supports Care Management staff, members and external providers to achieve member goals. This work is carried out in support of the mission and goals of Independence Care System. This is a full-time, permanent position.
Key Responsibilities:
1. Coordination of home care services
- Initiates home care services for all members including verifying all home care services prior to enrollment as well as identifying and assigning cases to PCA Providers. Verifies PCA services initiated with members.
- Coordinates and manages continuous home care services including changes, stops and re-start home care scheduled hours, creating referrals/authorizations and stop/changes.
- Identify and resolve home care issues/concerns that impact service delivery in a timely manner.
- Monitors activities related to utilization of home care hours.
- Assists home care workers in transferring to providers.
- Monitor and ensure Personal Care Aide Providers follow the appropriate processes for home care services.
- Review/initiate changes based on the 180 day member assessment completed by RN.
2. Documentation
- Effectively communicate and accurately document all case related changes, issues, incidents and actions into database.
- Ensure Personal Care Aide Providers receive appropriate documentation for PCA assignments and changes.
- Conduct investigations and provide documentation for grievance process.
3. Collaboration and Follow-up
- Collaborate with and supports Care Management staff, members and providers to achieve member goals.
- Coordinate with Care Management staff to facilitate member case conferences
- Maintain efficient process to ensure continuity of care for members.
- Communicate with members and Personal Care Aide Providers as necessary.
Minimum Qualifications:
- High school diploma or equivalent required.
- Minimum 2 years of relevant experience in a small to medium sized organization.
- Proven track record of customer service and multi-tasking skills.
- Bilingual – (English/Russian).

Team Leader
The Team Leader is responsible for managing the activities of the team and the performance of each team member. The Team Leader oversees the team in the coordination of members’ overall long-term care which includes a range of health and social supports aimed at maintaining health; preventing the exacerbation, of serious medical conditions; and promoting independent living in the community. This work is carried out in support of the mission and goals of Independence Care System.
Key Responsibilities:
1. Lead an Interdisciplinary Team
- Create an environment where a group of individuals with diverse training and backgrounds work together.
- Assure that team members consistently collaborate to solve member problems that are too complex to be solved by one discipline or many disciplines in sequence.
- Assure that team members work interdependently to define and create interdisciplinary care plans that address the members problems; and learn to accept and capitalize on disciplinary differences, differential power and overlapping roles
- Facilitate team meetings by discussions through the sharing of information and knowledge, identification of teamwork issues, development of problem‐solving recommendations, and standardizing team operations.
- Synthesize the different team members’ contributions and produce proposals that are easily understandable by all.
- Facilitate team members’ work on a day to day basis while at the same time keeping the “broader picture” in mind and thinking ahead.
2. Oversight of Team Responsibilities
- Assure completion of all team tasks within the unit using administrative reports, which include 180 day assessments, care plans, advance directives, monthly contacts, complexity assessments, chart reviews, and required documentation. The Team Leader monitors these functions to assure compliance with both ICS policies and DOH regulations.
- Assure that team members work collaboratively to improve member’s health. This includes respond quickly to assist members who are hospitalized or experience a change in their health and work to ensure that all ensuring members are linked to health care providers and that the team members work together to improve heal
- Assure that team members complete all required documentation (paper and/or computer based), to be compliant with contracts, regulations (Dept. of Health and contracted Providers), and/or ICS policies. Monitor this documentation for quality.
- Direct Team Members to assure investigation of member grievances and seek solutions that prevent repeat occurrences.
- Monitor Team Members ability to provide Care Coordination with members and implement and follow through on the care planning process.
3. Managing Development of Team Members:
- Coach and facilitate work of Team Members in the unit to promote individual performance and quality of service delivery to members. Assure that staff receives the guidance, support and direction that they need to be successful in their roles. Supervisory activities take place both in individual and group settings and include both office and community based work.
- Collaborate with appropriate staff members to provide orientation and training to newly hired team members, and to assess for quality of performance for all staff.
- Identify and implement professional development opportunities for team members.
- Support a work climate that encourages the collaborative efforts of team members and resources, working together to best meet the needs of ICS members.
- Create and maintain a work climate that supports staff retention.
- Identify strengths and stretches of team members and is able to communicate them to staff.
4. Organization/Management
- Work with the Vice President of Care Management to manage staffing within the unit. Assign members to team members, with attention to the needs of the member and the skill / experience level of staff.
- Maintain up to date knowledge of internal support resources and external resources to meet member needs. Demonstrate ability to access resources appropriately in support of member’s care plan.
- Implement care management policies and procedures and assure that Care Managers are educated and understand their role in implementation. Initiate and participate in the development and / or revision of policies and procedures as needed.
- Provide care coordination for a limited, active caseload. This work includes all of the essential functions of the team.
- Monitor status of the team, produces accurate monthly report which synthesizes data, chart reviews and team work. Identifies issues of concern, identifies plan for resolution and communicates to supervisor.
- Provide reports and documentation which demonstrate understanding of purpose and incorporates relevant information, and identifies issues and problems.
- Lead activities related to compliance, such as IPRO. Analyze reports for compliance measures and service utilization. Identify issues of concern and follows through with plan to address. Keeps supervisor informed of issues.
- Manages service utilization within the unit to ensure alignment with ICS fiscal structure.
5. Other duties as necessary.
Minimum Qualifications:
- Registered Nurse with Bachelor’s Degree in Nursing, licensed to work in New York State or;
- Master’s Degree in Social Work, Business, Public Administration or similar degree.
- Minimum 7 years’ experience in the social or health services fields.
- Minimum 4 years’ experience in a supervisory role.
- Minimum 2 years’ experience with the physically disabled population.
- Program management/administration experience.
- Experience in roles with progressive responsibility.
- Proven track record of advanced verbal and written communication skills.
- Comfortable with travel in Brooklyn, Manhattan, the Bronx, and/or Queens.
Additional Qualifications:
- Certification and/or license in field of discipline preferred.

Social Worker
As part of the Care Management team the Social Worker participates in assessing and evaluating the needs of members, and works with the member and the rest of the team in planning and implementing care that addresses the care needs of the member. This position is based in the community approximately 60% of the workweek. This work is carried out in support of the mission and goals of Independence Care System.
Key Responsibilities:
- Assess member’s status and needs, conduct onsite visits, as needed.
- Completes Comprehensive Care Plans – develops a care plan tailored to meet member’s needs, issues and desired outcomes.
- Collaboration – Participate with other team members in developing and monitoring Member care plans, including participation in care team meetings.
- Documentation, Reporting and Compliance – complete documentation, and reports that adhere to timeframes and ICS guidelines. Document care plan in member’s record, member contacts, and revise written care plan as needed.
Minimum Qualifications:
- Bachelor’s in Social Work (BSW) required. Master’s Degree in Social Work (MSW) preferred.
