Billing Instructions for FIDA-MMP Participants
All claims for Community Care Plus FIDA-MMP participants—apart from dental and vision claims—should be submitted to our billing coordinator Public Consulting Group (PCG) either electronically or by mail. Please note: Providers can determine if an ICS member is part of the FIDA-MMP or the MLTC by reviewing their ICS Service Authorization Form.
We encourage providers to submit claims electronically via the Electronic Data Interchange (EDI) process when possible, as electronic claims can be processed more efficiently and payments made more quickly. In order for claims to be routed to Independence Care System, please be sure to include our payor ID: 64284. This pin is the identifier at the clearinghouse to route claims directly to Claims Operations Department.
Paper claims for Community Care Plus FIDA-MMP participants—apart from dental and vision claims—should be submitted on either a UB-04, HCFA 1500 claim form directly to PCG at the following address:

Interdisciplinary Teams in Brief
Independence Care System (ICS) appreciates the care and services that our network providers offer through our plans.
As a provider in our FIDA-MMP network, you may be required by the State of New York to join the interdisciplinary team (IDT) that oversees care for all plan participants. This is particularly the case for primary care providers. New York State requires that all primary care providers seeing patients through a FIDA-MMP join those patients’ IDTs or designate someone from their practice to do so.
