Pharmacy Benefits

As a participant in ICS Community Care Plus FIDA-MMP, you have access to comprehensive medication and pharmacy benefits. Below, you will find the most up-to-date drug list, and you can learn all about your benefits. For more information call ICS at 1-877-ICS-2525, Monday through Friday, from 8 a.m. to 8 p.m.

2017 Drug List

List of Covered Drugs (Also available in Spanish, Russian, Chinese, Haitian (Creole), Korean, and Italian.) The Drug list is a list of drugs covered under the FIDA-MMP Plan. You can search for your medication by drug name (in alphabetical order) or by condition (i.e. diabetes). You can quickly search the PDF document by following these steps:

How to Search a PDF Document

If you are looking for a specific topic in a long PDF document:

  1. Open the document in your PDF reader. In most cases, this can be done by clicking on the file or link.
  2. For a quick search, do one of the following: Select Edit > Find in the main menu, or Press “Ctrl-f” (“Command-f” or “Apple-f” on a Mac). Tip: “Ctrl-f” opens the Find function in most applications, including browsers and Microsoft Office applications.
  3. Enter the word or phrase you are looking for in the form field provided and press “Enter” or “Return”.

In most applications, the first instance of the word or phrase in the document will be highlighted. Scroll Through Search Results You jump to the next instance of a word or phrase that you have searched by clicking on the right arrow found next to the search box or pressing “Ctrl-g”. Likewise you can return to an earlier instance using the left arrow or pressing “Shift-Ctrl-g”. ICS Community Care Plus FIDA-MMP participants can use our on-line drug list look up tool by signing in to the member portal.

There are effective, lower-cost drugs that treat the same medical condition as this drug. You may be required to try one or more of these other drugs before the plan will cover your drug. If you have already tried other drugs or your doctor thinks they are not right for you, you or your doctor can ask the plan to cover this drug.

The plan requires you or your doctor to get prior authorization for certain drugs. This means the plan needs more information from your doctor to make sure the drug is being used correctly for a medical condition covered by Medicare. If you don’t get approval, the plan may not cover the drug.

The Medication Therapy Management (MTM) program is a free program offered by ICS Community Care Plus FIDA-MMP. Plan participants that take multiple medications or medications for chronic illnesses that are largely controlled by medications may be eligible for this program. The goal of the program is to help you maximize the effectiveness of your medications and to ensure you are taking them safely and properly. Our pharmacist will work with  you and your doctor to make sure all of your medications are working for you, that you are taking them as prescribed and that your are not experiencing any negative side effects.

If you are eligible for the program, you will be notified by mail or by phone. The program is voluntary. If you do not wish to participate in the program, it will not impact your current prescriptions or your ability to get your medication. If you do participate in the program, you will be contacted by our pharmacist on a schedule that you agree to at the beginning of the program. You will be asked to complete two forms: the Personal Drug list, a list of your medications that you will update as it changes, and Medication Action plan, a document that helps you to take your medications regularly as prescribed.
