Welcome to the ICS Community!
As a participant in ICS’s Community Care Plus FIDA-MMP, you have access to an exceptional network of doctors, specialists, hospitals, pharmacies, and long-term care service providers, as well as to ICS staff specialists in multiple sclerosis, spinal cord injury, women’s health, wound prevention, disability, dementia care and more, all working together to keep you healthy and safe at home.
Our ICS Community Care Plus FIDA-MMP Participant Handbook will tell you everything about how your plan works, but we wanted to give you some quick tips here.
When I join ICS, what will I receive?
When you join ICS, you will receive a letter in the mail welcoming you to the ICS community. You will also receive a Participant ID card that you will be able to use to access the services available to you as an ICS Community Care Plus FIDA-MMP participant. If your provider is not familiar with ICS Community Care Plus FIDA-MMP, you can give them a copy of this letter to help explain more about our plan.
Who helps me get the services I need?
Your very own Interdisciplinary Team (IDT)
This is a group of people that you help put together; it’s your team and you are a member of the team. The people will get to know your needs and work with you to develop and carry out your own “Person-Centered Service Plan.” Your IDT may include your care manager, doctors, service providers, and other health professionals. ICS staff who support your IDT
These ICS staff include nurses, senior home health aides, rehab and wheelchair services staff, and care management coordinators who work with your IDT.
ICS Call Center Staff
While your care manager is ultimately responsible for coordinating your services with you and your IDT, our Call Center staff are on hand 5 days a week, Monday through Friday, from 8 a.m. to 8 p.m., to handle your day-to-day needs. They can order your supplies, make your referrals, help to arrange your transportation, make appointments for wheelchair services, and more. If you need something they can’t handle, they will make sure to put you in touch promptly with your care manager or a care management coordinator. What if I disagree with decisions about my services?
There are actions you can take.
You have the right at any time to file a grievance or to appeal a plan of action. Also, you can disenroll from ICS at the end of any month. ICS has policies and procedures [scroll to the bottom of the page] in place to protect your rights.
You can also contact the Participant Ombudsman.
The State of New York has created a participant ombudsman program called the Independent Consumer Advocacy Network (ICAN) to provide Participants free, confidential assistance on any services offered by ICS Community Care Plus FIDA-MMP. ICAN may be reached toll-free at 1-844-614-8800 or online at icannys.org.

We are proud to offer a wide range of classes and special events that will help you to become an active, engaged and—we hope—happy participant with ICS! You can choose from more than 15 weekly and bi-weekly activities, from art classes to writing groups, as well as seasonal events including trips to sporting matches, art exhibitions, and much more! - Join the ICS Participant Advisory Council! Your feedback, concerns, and ideas are important to us. ICS has long had an active Participant Advisory Council—all ICS participants are invited to attend meetings and get involved.
