Transportation Coordinator
The Transportation Coordinator is responsible for ensuring successful travel arrangements are established and delivered, between ICS providers and members. This work is carried out in support of the mission and goals of Independence Care System. This is a full-time, permanent position.
Key Responsibilities:
1. Coordinate transportation service for members. This includes, but is not limited to:
- Responding to all requests for transportation arrangements via phone calls routed to the transportation hotline, retrieving messages from the hotline voicemail box, emails to the transportation email inbox and alerts from ICS database.
- Confirming with members and facilities, medical appointments, referral confirmations and hospital discharge authorizations.
- Entering accurate travel request documentation in ICS database, and notifying provider of request, via approved method of notification.
2. Confirm daily trip list with all participating providers.
3. Troubleshoot member issues and advise Transportation Manager of emergencies or discrepancies, escalate member issues as appropriate.
4. Engage in the creation and/or maintenance of reports, such as standing appointments, no show appointments, provider transitions.
5. Process member livery applications.
6. Translate correspondence received from members and/or outgoing correspondence as necessary.
7. Record Transportation Hotline Messages as necessary (i.e. holidays, emergency closings).
Minimum Qualifications:
- High School Diploma or GED a must. Undergraduate degree preferred.
- Excellent communication, interpersonal, organizational and motivational skills.
- Computer literacy in a network environment with a Windows operating system.
- Familiarity with Medicaid, managed care, health care delivery a plus.

Care Manager RN, Senior Adult Team
As part of the Care Management Team, the Care Manager, RN participates in assessing and evaluating the clinical needs of members, and works with the member, family caregivers and the rest of the team in planning and implementing care that addresses the member’s care needs. This work is carried out in support of the mission and goals of Independence Care System.
Key Responsibilities:
- Conduct Assessments – conducts a history and physical assessment. As well as re-assessments and other clinical assessments as appropriate.
- Assess member/caregiver knowledge of necessary care needs such as medication management, DME use and safety measures
- Collaborate with other team members and the member to establish a comprehensive care plan that identifies all needs.
- Facilitate discharge planning when necessary.
- Document all findings and member related communications in database.
- Ensure compliance with care guidelines and protocols.
Minimum Qualifications:
- RN with current NYS licensure.
- Minimum two (2) years relevant work experience.
- Documented assessment, care planning and communication skills.
- Highly self-motivated and ability to manage time independently.
- Comfortable with extensive travel in Brooklyn, Manhattan, the Bronx, and/or Queens.

Nurse Educator
The Nurse Educator collaborates with the clinical supervisors, unit leaders, care managers, member services, internal and external resources and members to build a system of care that supports the member’s full participation in community life. This work is carried out in support of the mission and goals of Independence Care System.
Key Responsibilities:
1. Track Performance of Nursing Staff
- Responsible for planning, developing, and implementing tools to measure nursing competency.
- Monthly reporting on performance measures.
2. Development of Nursing Staff
- Report findings of staff performance to Clinical Supervisors and assist them in creating a plan to meet individual training needs and improve performance.
- Provide guidance, coaching and supervision to nursing staff.
- Plan and develop trainings based on needs of nursing staff.
- Develops, implements, monitors and evaluates/improves the orientation, cross training and education/competency processes/systems for nursing staff.
- Responsible for the continuous quality improvement of ICS nursing department.

3. Documentation and Compliance
- Perform field visits with nursing staff to assess and evaluate individual performance according to ICS policies and procedures, state and federal regulations, and current nursing standards.
- Ensures compliance and documentation of member records based on ICS standards by routinely conducting internal audits.

Minimum Qualifications:

- Minimum 5 years of work experience in a health care setting, preferably community health.
- Minimum 2 years of experience in management/leadership roles with responsibility for direct supervision of staff.
- Creative and collaborative approach to problem solving.
- Excellent interpersonal skills, sensitivity to cultural and personal diversity.
- Advanced verbal and written communication skills.
Additional Qualifications:

Receptionist
As part of the Administrative staff, the Receptionist performs various duties to support the office operations of ICS’ Brooklyn office. This is a full-time, permanent position.
Key Responsibilities:
- Reception – Answer phone calls; respond to inquiries about ICS and transfer calls as appropriate. Receive and greet members and/or visitors and direct them to appropriate person, meeting or event. Maintain lobby area presentable and welcoming.
- Office Administration – Ensure day to day administration of office and act as point person for all office and facilities matters, escalating issues as necessary to Office Manager for resolution. Maintain ICS conference room schedule in Meeting Planner.
- Supplies and Office Equipment – replenish supplies and conduct preventive maintenance on ICS office equipment, troubleshoot failures and arrange repairs as necessary.

Member Services Coordinator, ISU
As part of the Information and Solution Unit, the Member Service Coordinator receives member telephone calls, and works with all departments in ICS to facilitate timely response to member needs/concerns.
Key Responsibilities:
- Responsible for answering and triaging all inbound telephone calls.
- Research and problem solve issues/inquiries, including contacting Care Managers and other units as needed in order to provide member’s with requested information or to resolve problem.
- Monitor all open items, escalating to supervisors as necessary.
- Keep members updated as to the status of any open issues/items.
- Assist and encourage members in scheduling appointments and arranging transportation and other services as necessary.
- Send potential member leads to the Intake department when necessary.

Wound Care RN
As part of the Wound Care Team, the Wound Care RN participates in assessing and evaluating the clinical needs of members with wounds, and works with the member and the rest of the team in planning and implementing care that addresses the member’s care needs. Key Responsibilities:
- On-going monitoring of all members with wounds.
- Coordination with member’s Social Work Care Manager.
- Coordination with home health agencies and all other health care providers.
- Obtaining and reviewing skilled nursing notes from home care agencies (monthly).
- Completion of the Braden and Wound Assessment Tools in Care Compass for all members with open wounds.
- Conducting 180 day and SCIC assessments and completing all related paperwork.
- Completing the wound care section of the member’s care plan.
- Accompany member to physician/ wound care center appointments as needed.
- Coordination with Wound Care Program Director as necessary.
Minimum Qualifications:
- RN with current NYS licensure
- Track record of documented assessment, care planning and communication skills
- Experience with disability or rehabilitation programs helpful
- Good computer skills, including Microsoft Office work and Excel

Manager, Paraprofessional Services
The Manager, Paraprofessional Services oversees day to day operations of the Paraprofessional Services Department. This work is carried out in support of the mission and goals of Independence Care System. This is a full-time, permanent position.
Key Responsibilities:

- Supervise, support, and evaluate PCA Coordinators activities and performance

Member Services Coordinator (Pilot Team)
As part of the Care Management Pilot Team, the Member Services Coordinator will perform functions that support the Care Management process and team members. This work is carried out in support of the mission and goals of Independence Care System. This is a full-time, permanent position.
Key Responsibilities:
1. Collaborate with ICS staff members and providers to ensure that our member’s needs are being met.
- Arrange for services initiated by the care team.
- Provide administrative support to the team
- Assist in reconciling claims versus referrals for billing and payment purposes.
- Process all service plan referral requests.
- Function as a liaison between provider and member as appropriate.
2. Assist in ensuring compliance by:
- Acquiring signed Medical Doctor (MD) orders
- Scheduling visits arranging escort

5. Responsible for Documentation
- Document dates of receipt of service plans and SAAM submission.

Manager, Provider Relations
The Manager, Provider Relations is responsible for executing the strategic direction of the organization’s provider network. This work is carried out in support of the mission and goals of Independence Care System. This is a full-time position.
Key Responsibilities:
1. Credentials & Auditing
- Conduct audits, satisfaction surveys and other performance improvement activities related to ICS providers as appropriate.
- Conduct accessibility audits of site-based providers and prepare response for providers.
- Conduct all credentialing and re-credentialing activities
2. Collaboration
- Conduct contract negotiations with new and existing providers in conjunction with Director of Administration.
- Collaborate in and facilitate development of standards of care with ICS staff and providers.
- Collaborate in policy and procedure development as it relates to provider relations.
3. Provider Network
- Identify and investigate new providers for participation in ICS provider network.
- Evaluate and analyze effectiveness and efficiency of provider network, identifying gaps in service and /or inadequate capacity.
- Develop process maps to meet operational needs related to provider relations.
- Participate in ICS staff education and orientation efforts related to provider relations.
- Promote and maintain positive and professional relationships with participating providers.
- Initiates problem solving activities in response to member, staff or provider complaints, billing or payments discrepancies. Collaborates, implements and monitors corrective action plans as appropriate and at the direction of Director of Administration.
- Facilitate the inclusion of providers requested by membership into the provider network as appropriate
- Develop, distribute and analyze data relevant to provider activity and prepares reports for senior staff as appropriate.

PCA Service Coordinator
The PCA Service Coordinator initiates home care services for all members, and coordinates/monitor continuous home care services. S/he maintains an efficient process to ensure the delivery of high quality continuity of care for members. The PCA Service Coordinator collaborates with and supports Care Management staff, members and external providers to achieve member goals. This work is carried out in support of the mission and goals of Independence Care System. This is a full–time, permanent positition.
Key Responsibilities:
1. Coordination of home care services
- Initiates home care services for all members including verifying all home care services prior to enrollment as well as identifying and assigning cases to PCA Providers. Verifies PCA services initiated with members.
- Coordinates and manages continuous home care services including changes, stops and re-start home care scheduled hours, creating referrals/authorizations and stop/changes.
- Identify and resolve home care issues/concerns that impact service delivery in a timely manner.
- Monitors activities related to utilization of home care hours.
- Assists home care workers in transferring to providers.
- Monitor and ensure Personal Care Aide Providers follow the appropriate processes for home care services.
- Review/initiate changes based on the 180 day member assessment completed by RN.
2. Documentation
- Effectively communicate and accurately document all case related changes, issues, incidents and actions into database.
- Ensure Personal Care Aide Providers receive appropriate documentation for PCA assignments and changes.
- Conduct investigations and provide documentation for grievance process.
3. Collaboration and Follow-up
- Collaborate with and supports Care Management staff, members and providers to achieve member goals.
- Coordinate with Care Management staff to facilitate member case conferences.
- Maintain efficient process to ensure continuity of care for members.
- Communicate with members and Personal Care Aide Providers as necessary.

Administrative Assistant

Assessment RN
The Assessment Nurse, as part of the Care Management Department is responsible for conducting comprehensive and accurate assessments in all boroughs in which ICS serves members. This is a permanent, full-time position.
Key Responsibilities:
- Assessments – conduct the following assessments: initial, 180 day, post-hospital, SCIC (significant change in condition), and assessments required as part of the Grievance and Appeals process. For all assessments, complete the accompanying documentation required by ICS Policy and Procedure.
- Member Relationship and Education – establish a working collaborative relationship with members; assess member/caregivers’ knowledge of necessary care requirements, i.e. medication management, new onset diagnosis, DME use and safety measures.
- Documentation – complete thorough documentation as per established policy and Procedure; assist with the coordination and collaboration of the care plan with the Care Manager and the member; prior to making an assessment visit, review member record to familiarize self with members’ current problems/issues/needs, and progress towards goals.
Minimum Qualifications:
- RN with current NYS licensure.
- Prior experience documenting assessments and care planning.
- Computer literacy in a network environment with a Windows operating system.
- Ability to travel within all NYC boroughs.

Compensation:
This position requires travel to member’s homes in every borough which ICS serves – this position will receive an additional 5% to the base salary.

Care Manager, Social Worker
Position Overview:
Key Responsibilities:
- Conduct psycho-social assessments, including assessments of home environment, financial situation, living situation, family and community supports, preferences, strengths, perceived medical and home care needs, and understanding of diagnoses and disabilities.
- Participate with other team members in developing and monitoring Member care plans, including participation in care team meetings.
- Explore and monitor financial options and eligibility for various benefits.
- Provide information and assist members with coordination of housing issues, mental health, and/or alcohol or drug abuse.

- Provide ongoing coordination of psycho-social services.
