Care Manager, Social Worker
Position Overview:
As part of the Care Management Team, the Social Worker participates in assessing and evaluating the psycho-social needs of members, and works with the member and the rest of the team in planning and implementing care that addresses the care needs of the member.
Key Responsibilities:
- Conduct psycho-social assessments, including assessments of home environment, financial situation, living situation, family and community supports, preferences, strengths, perceived medical and home care needs, and understanding of diagnoses and disabilities.
- Participate with other team members in developing and monitoring Member care plans, including participation in care team meetings.
- Explore and monitor financial options and eligibility for various benefits.
- Provide information and assist members with coordination of housing issues, mental health, and/or alcohol or drug abuse.
- Provide information and assist members in maintaining and establishing community links.
- Provide ongoing coordination of psycho-social services.
- Provide supportive counseling for crisis intervention, as appropriate.
Minimum Qualifications:
- BA required; BSW or MSW preferred.
- At least two years of experience in a social services or health care environment.
- Knowledge of health care and disability issues and systems.
- Comfortable with travel in the boroughs of Brooklyn, Manhattan and/or the Bronx.
Additional Qualifications:
- Experience in healthcare and/or nonprofit a plus.
- Bilingual: English/Spanish capability is preferred.

Manager, Intake & Enrollment
Position Overview:
This position is responsible for the managing/coordination of the intake and enrollment process from the point of referral until member enrollment. Additionally, the position will be responsible for managing the Intake Department Operations which includes collecting and coordinating all necessary demographic and clinical information necessary to qualify and complete the admission process of members.
Essential Functions:
- Manage day-to-day operations of the Intake Department including: Review each initial referral and assess its appropriateness for the next steps in a timely manner.
- Supervise the Intake Department staff and conduct weekly process meetings to address procedural issues.
- Track key measures, including referrals and sources, referrals processed, assessments completed, enrollments and exited without enrollment.
- Monitor, track and report on work of Medicaid Eligibility Specialists, including number of new applications and re-certifications.
- Create and maintain a monthly report to inform Director, Administration on the number of cases opened, number of cases pending, number of cases completed.
- Identify eligibility and enrollment problems, bring them to attention of supervisor, and participate in process of developing and implementing ways to address them.
- Ensure that confirmation letters are sent to prospective enrollees when they withdraw, do not meet the criteria or indicate at the assessment that they are not interested.
- Purge active intakes each month by identifying impediments to enrollment and addressing those impediments or exiting applicants.
- Monitor Intake Tracking process and address issues hindering enrollment in a timely manner.
Minimum Requirements
- At least five years of work experience as an administrator or in an administrative capacity.
- Bachelor’s degree required.
- Previous leadership experience in administration, operations and/or program services.
- Extensive knowledge working with city and state agencies around benefits such as public assistance, SSI, Medicare, Medicaid.
- Strong administrative skills combined with social service acumen.
- Excellent interpersonal skills.
- Ability to manage complex process of intake and enrollment in a fast-paced environment.
- Bilingual (English/Spanish) required.
Additional Requirements
- Bachelor’s degree in Psychology, Sociology or Social Work preferred.
- Experience working directly with people with physical disabilities preferred.
- Previous work experience in healthcare and/or non-profit preferred.
- Experience working in a multi-site organization a plus.
